I was really impressed by this simple mnemonic which helps with an excellent handover.

SHIFT method [learnt during an MPS(medical protection society) meeting,altered the contents slightly to suit ITU handovers]

S tatement (e.g.69 year old male patient presenting in type 2 respiratory failure.),

H istory, (relevant)

I nvestigation (significant ones and pending), 

F ocus(fears/concerns),

T reatment (pending).
